Refundable accommodation deposit application form

Use this provider form to apply for approval to charge a refundable accommodation deposit (RAD) (or equivalent daily amount) that is more than the current maximum accommodation payment amount prescribed by the Aged Care Rules 2025.

Use the RAD application form to record the information for accommodation groups 1 – 6. 

Use the supplementary form only to record information for accommodation group number 7 and onwards. 

Aged Care Act and other changes 

We have updated our application form to align with the new Aged Care Act 2024 and to provide better guidance for providers during the application process.  

Key changes include: 

  • updating terminology, service references and legislative references
  • including more detail on how to provide information that aligns with assessment requirements under Division 2, Subdivision B of the Aged Care Rules 2025
  • updating how we ask about extra service fees including higher everyday living fees 
  • enhanced privacy statement and sharing of information declaration.
